Thank you for choosing Sinclair Health Clinic for your healthcare needs. When you schedule an appointment with us, we set aside enough time to provide you with quality care. Please contact us as soon as you know you need to cancel or reschedule your appointment.

If you miss three consecutive appointments or three within a three-month period without giving any prior notification to Sinclair Health Clinic, you will be temporarily discharged from our practice for six months. After this six-month period, you may reapply to our practice for continued care.

If labs are required for your appointment, they must be completed at least seven days prior to your appointment. This will allow our providers time to review the results before your scheduled visit.
